Premera & LifeWise Member Information

If you are a Premera or Lifewise member of the networks listed below, you are required to call the number on the back of your ID card to double check your eligibility, co-payment, deductible, and any changes to your insurance at the time of services.  We cannot confirm your benefits for you.  It is the client’s responsibility to understand their benefits and it shouldn’t take more than about 10 minutes!  Greenlake Wellness Group will then do all of the billing from there.

When calling, DO NOT INQUIRE USING YOUR THERAPIST’S NAME!  Greenlake Wellness Group is what you’ll need to ask about, NOT an individual’s name.  As long as Greenlake Wellness Group is for sure in-network for your outpatient mental health benefits, then all of the clinicians who work for us are included.  Asking about an individual therapist will only lead to confusion between you and the membership services person you speak with!

Unfortunately, we cannot bill insurance for couples, family, or group sessions.  Couples and families would need to pay out of pocket.  We CANNOT provide you with a Superbill to get reimbursed if you are in network.

When calling, these are the questions you’ll need to ask so you can confirm your benefits:

  • Does my “outpatient mental health coverage” apply at Greenlake Wellness Group?
    • We bill as a professional service NOT as a facility.
  • What is my deductible and has it been met?
  • Do I have a co-pay and/or co-insurance for each office visit?
  • What percentage of my therapy fees are covered?
  • How many therapy sessions are covered per year?

If you begin seeing one of our therapists and insurance rejects claims for these sessions because you are “out of network”, you may be required to pay our cash prices via credit card for all of the sessions that have already occurred.  Greenlake Wellness Group and it’s clinicians are not responsible for interpreting or understanding your benefits!

*our cancellation policy requires notice at least 48 hours before your scheduled appointment.  For in network patients, an un-billable $120 fee will be charged to the card on file for late cancellations or no-shows.

Once your eligibility is confirmed as IN NETWORK with Greenlake Wellness Group, we can get you scheduled ASAP!  If you haven’t already been in touch with our office manager, please reach out using the contact form below.
