Checklist for new GWG Therapists
  1. Upload all your information for our records Professional Documents Upload - same password as this page.

  2. Insurance

    1. Pending Associate License (No license issued yet)

      Who this applies to:

      • You have graduated.

      • You have submitted your associate license application to Washington DOH.

      • Your license is in "Pending" status.

      What you need to do:

      1. Obtain your NPI number (if you do not already have one).

      2. Submit your associate license application to Washington DOH.

      3. Send GWG (both in Professional Documents Upload):

        • NPI number

        • Proof of associate license application submission (HELMS screenshot or confirmation email)

      4. GWG will submit a Provider Update Form to Premera to add you to our contracted group.

      5. You may see Premera clients under supervision only after GWG confirms you have been added to our Premera contract.

    2. Associate License Issued (LMHCA, LMFTA, LASWA, etc.)

      Who this applies to:

      • You have received your Washington associate license.

      What you need to do:

      1. Send GWG:

        • License number

        • License issue date

      2. Create a CAQH profile if you have not already done so.

      3. Fill out the GWG Insurance Form to be linked to GWG's group profile in CAQH.

      4. GWG will submit a Provider Update Form to Premera to add you to our contracted group.

      5. You may see Premera clients under supervision only after GWG confirms you have been added to our Premera contract.

    3. Fully Licensed (LMHC, LMFT, LICSW, etc.), Uncredentialed

      Who this applies to:

      • You have received your full independent license and have not been credentialed with insurance.

      What you need to do:

      1. Follow these instructions in order to be partnered with insurance. They take 1-2 months to get credentialed once filled out, so please prioritize: Premera Provider Credentialing Instructions

      2. Send GWG:

        • Full license number

        • License issue date

        • Confirmation that CAQH is complete and current

        • Initial credentialing date with Premera once known

      3. Fill out the GWG Insurance Form to be linked to GWG's group profile in CAQH.

      4. You may see Premera clients only after GWG confirms you have been added to our Premera contract.

    4. Fully Licensed (LMHC, LMFT, LICSW, etc.), Credentialed

      Who this applies to:

      • You have received your full independent license and have already been credentialed with insurance.

      What you need to do:

      1. Send GWG:

        • Full license number

        • License issue date

        • Confirmation that CAQH is complete and current

        • Initial credentialing date with Premera

        • Fill out the GWG Insurance Form to be linked to GWG's group profile in CAQH.

      2. GWG will submit a Provider Update Form to Premera to add you to our contracted group.

      3. You may see Premera clients only after GWG confirms you have been added to our Premera contract.

  3. Therapist preferences Find your name on the bottom tab and fill out.

  4. Sign the Docusign forms that will be emailed to you:

    1. Professional Service Agreement (Contract)

    2. Business Associate Agreement (HIPAA)

    3. Direct Deposit Authorization (direct deposit hits on 1st and 16th of each month)

    5. Secondary Supervisor - Julia Jenkins - for Insurance



At Greenlake Wellness, our billing team handles all client billing and insurance processing so therapists can focus on clinical care.


Billing & Outstanding Balances

If a client has an unpaid balance, you may be copied on an email from Katherine notifying the client of the outstanding amount.


We recommend not rescheduling clients with unpaid balances until payment has been made. Please note that therapists cannot be paid for sessions until the client’s balance has been paid.


Our billing team will continue following up with the client directly regarding payment. It is ultimately your decision whether or not to continue seeing clients with outstanding balances.


Autopay

All clients are enrolled in Autopay through SimplePractice. Clients select and save a credit card within their SimplePractice portal.


At the end of the day, SimplePractice automatically charges the client for their session balance.


For clients using insurance:

  1. Insurance is billed first.

  2. Once the insurance claim is processed and paid, SimplePractice automatically charges the client for any remaining copay, coinsurance, or deductible amount.


Important Note for First-Time Insurance Clients

For new clients using insurance, it can take approximately 2–3 weeks for the initial insurance claim to be processed and returned. During this time, we may not yet know the client’s exact financial responsibility, including copays, coinsurance, or deductible amounts.


Once the insurance claim has been processed, SimplePractice will automatically charge the client for any remaining balance owed.


Our billing team monitors these claims closely and will communicate with clients regarding any outstanding balances as needed.


This entire process is managed by our billing team. Our goal is to make sure therapists are paid accurately and on time.


Out-of-Network Clients & Superbills

For out-of-network clients, we can provide superbills for insurance reimbursement.

Clients should contact us directly if they would like superbills so we can enable superbill settings in SimplePractice.


We can also create customized superbills with additional information required by the client’s insurance company. These are manually prepared by Katherine as needed.
